Nevermind the previous question, I'm going to work on this from a different angle: what are some popular examples of an nds rom that cannot be extracted then rebuilt and still function? Or should these basic steps always produce a functional rom?

My basis is this: working with PSP isos, they sometimes have an index called the LBA which dictates that a file will be found in a specific location, and if this changes then the iso is corrupt. Rock Band Unplugged is one example. I'm just not certain if DS has something similar.

I have dslazy 0.6 but the rom I was working on wouldn't ever work after my attempts to edit it (white screens on the DS). Finally I decided to simply unpack then repack it as-is to see if it works at all, and it does not. So either this is specific to not being able to modify the particular game, or I need a better tool and I'm assuming the latter.

Try dsbuff? I think it's basically just the updated version of dslazy, but it's worth a try:

Yeah I remembered that dslazy is just a frontend for ndstool in the first place, so hopefully there is a newer version and that's the only issue.
 
To my knowledge it is not anything like the PSP but more that NDStool is broken (it was only ever intended for homebrew but worked well enough for earlier hackers) hence us having the various other programs we have today (nitro explorer being one http://treeki.shacknet.nu/ of the more general tools, ndsts, ndshv and crystaltile2 being a few more). Games that fail are New Super Mario Brothers, one of the tony hawks games (possibly the first) and after that if something fails an undub it is usually the fault of ndstool (stuff like http://gbatemp.net/t231016-witch-s-wish-undubing is very much the exception along with the sound file being tweaked/rebuilt between releases and falling over thanks to that which is equally rare).

Possible exception- some AP can be triggered by this. Certainly when no$gba could still run most AP protected roms you could sometimes trigger the AP in no$gba with a rebuilt rom. I would sooner bet on it being a side effect of ndstool not being so good.
