He does the +2 and then during your opponents turn, all creatures are required to attack. It's an easy way to let your opponents cautious creatures get plucked off and leaves your opponent wide open for a strong attack, plus you can just kill one of their tapped creatures or slap 'em with a 6/6 Gideon.
I've been using Karn a lot lately though and he's been bossing games for me. If you get him out early (which I commonly do playing green ramp) I can just own for the rest of the game. Remove your opponents creatures from the game, exile their cards from their hand, and if you have a good enough defense, starting exiling Emrakul from your hand, do his -14, and restart the game with an Emrakul in play plus anything else exiled.
I was also playing some meh Blue/Green infect deck I through together a while ago and it was able to crush my friend's overpowered Goblins deck. Just countered all his Siege Gang Commanders and Goblin Chieftains and he couldn't even hit me with a lackey. Just used Blighted Agents to get him to 10 counters.
