Hacking GC Backup Loader Xi Error on 4.0

Specs:
System Menu 4.0, IOS249, IOS35, IOS36 installed. Used the latest MIOS patcher, got MIOS rev 10.

However, he gets this error on a disc. He's only tried one disc, it's a multiboot gc game made from GCOS out of a japanese game and 2 pal. However, he just gets this error and nothing else.

So, bad disc? Bad burn? System problem?

I'm getting him to burn a normal wii disc to see if that works.

it wont boot a wii disc if it is MIOS... burn a single GC game to see what happens. i find that multi game GC ISOs are dodgy for me, even though there is 2 on the disc it says there is one and wont let me swap... strange. but singles work fine.
