Revision 002 needs an FW above 3.0.0, specific fw version and game version is unknown to me for now

The important information is one of the digits on the back of the cartridge.

• DS: 6th character is the ROM revision.
• 3DS: 7th character is the ROM revision.

This information is also available in the cartridge header, though those aren't generally available at the moment for Switch games.

Based on the information in this thread so far:

• BotW with 1.0.0: AAAAA22A000
• BotW with 3.0.1: AAAAA22A001
• Pokkén with 3.0.0: BAAYA09B000
• MK8DX with older than 3.0.0: AABPA22A000
• MK8DX with 4.0.1: AABPA41A002

The pattern seems clear: the last digit in the cartridge's manufacturing code indicates the ROM revision.

EDIT: Looks like this was in the spoiler tags in the original post, which I didn't see. Leaving this post here for reference anyways.

Generally they include the current update at the time the ROM is sent to manufacturing. For example, my Smash Bros. 3DS cartridge is Rev.5 and has v1.1.0 preinstalled.

As far as I see there is no clear indication whether the cartridge contains an update or not. The only way to find out would be to start the update without any internet connection and cleared update cache.
There is however the Games required Firmware version thread which lists some games that contain an update like Pokken. It is a reasonable say that those games with never revisions also contain a new update.

Green for 1.0.0 - 3.0.0, Yellow for 3.0.1 - 3.0.2, Orange for 4.0.0 - 4.1.0, Red will be upcomming versions until we know that they are hb able.

For BotW you are out of luck. You have revision 001 which already needs 3.0.1. Try and get an used one from GameStop or some shop of this kind. Ask to check the backside for the last 3 numbers to be 000

To get this info, your console must be able to show you the game icon on the home menu. The rest is like explaned on the first post. select the game, press - on the left JoyCon and it will show the info of the game.
With "w/o online updates" i meant you to state the version without ever downloading an update to keep ppl from posting updated game versions.
