Your suggestion is very good. However Maybe we have the different idea on good english. Like we say "blessed are the poor in spirit", they don't know they are stupid, they live happily.
I think what you are missing in english is vocabulary.
If you are like me, you can read and understand almost everything correctly, but I can't write sentences by myself using advanced words (specific domains and themes, like laywer's terms, medical words, etc.), even polite or formal form to spkeak with your superior.
I suggest you read a lot of English texts or books to get used to vocabulary from "outside internet forums and chats". Internet is full of people not speaking proper grammar, so you may learn bad things (like a lot of people mix "then" and "than", "your" and "you'r" etc.).
What's great with people learning a language is that they are learning grammar at school and they tend to be more careful at choosing the correct word than native speakers. (same for people learning French and native French)
About oral, there are not a lot of possibilities.
Try watching movie/listening radio informations without subtitles. (good to understand, but not enough to speak).
Travel to an English speaking country few weeks/months.
Check if you have English speaking community near your town, like foreign students.

i have difficulty in creating sentences in english orally because there aren't many people who knows how to speak proper english here.
and in typing you can easily press backspace and type it again, or proofread it. whereas in speaking, you won't have the same amount of time to think about what you will say.
