Check (first of all) the SD card of your console, to discard any problem or damage with that:

• (Try to) make a backup of all its content.
• Do the quadruple reformat technique.
• Do an analysis with H2TestW.

For help to be given you really need to provide more information. Like is it a original 3ds or N3DS. Are you having issues with ligit bought eshop titles or are you running cfw and pirated games. What firmware is your 3ds? what version is your cfw ect ect. It's very hard to give help if we don't have all the information otherwise we are just guessing at what the issue is and how to fix it.

Edit It would also be helpful to know your sd card brand and size and what cluster size you used. And the cards read speed and so on.
