OP, you probably:
1 - Got a bad SD reader
2 - Got a fake microSD
3 - Got a defective microSD
1st, try using you PC+SD reader, go to disk management, delete the partitions, assign the entirety of the card to only one partition, reformat.
If it's too hard or isn't working try using
https://www.sdcard.org/downloads/formatter_4/
It will also force format the SD card to use the entirety of it's space.
After doing it, use h2testw to test your SD card actual capacity.
If it fails the test, try using another reading device (using you smartphone may do the trick, however you'll need it to use it as a mass storage device to copy files through an USB cable), if it still fails, it's either a defective microSD or a fake one.
Believe, the probability of you getting a fake microSD is way high with anything above 32gb, even if you bought it in some physical retailer.
