Is your Wii connected to the internet? I see you have IOS58 installed but you have an older version of HBC (unless that is just how sysCheck displays newer HBC versions)
Your cIOS is rather outdated, so a lot of newer games won't function correctly. You should download ModMii and use that to update your softmod, that will update everything to the latest (recommended) version.
You'd also want to install BootMii as boot2 if you haven't already (installing it again won't hurt if you're unsure, but it looks like you don't have it installed), this will protect you from bricks and allow you to backup/restore your NAND.

I don't think there's any way to find out through software. You could try every single chip update disc to see if it works, but that is not really a good idea.
Yeah, maybe you should open up the Wii, take pictures of the chip and upload them here if you don't recognize them yourself. The warranty is void anyway, and just opening it carefully should not damage anything.Aw... Yes, thats what I supose to say
