Probably. The manual of the 3DS does say to look at it at a distance of 14in. or so, or like 35cm(?)
The "Blur" is caused by your eyes not being able to combine the 2 images properly at close distances.

Is this unique to this title, what do other games/videos look like on your system

It's a common issue referred to as "crosstalk", or "ghosting", and I've found it's more noticeable on objects against a dark background (like the KH 3D menu). Most 3D implementations suffer from it, some less than others, but because the 3DS is autostereoscopic some images meant for one eye leak toward the other.

Sometimes you can improve it by adjusting the position of the 3DS (or your face), but you'll almost never eliminate it completely.

FYI you can try calibrating the 3D from Settings.

Does that actually do anything? I just thought it was more of a "test card" which let you vary the slider, just to look at the change in depth.. I don't think you can actually "calibrate" anything... But I could be wrong!

I dunno but once my 3D was blurry so I went to do that test again. It worked.

Use a lower brightness setting like 2 or 3 and don't play in a completely dark room or near a window where it's too bright, what kind of lightning conditions there are where you play have a big impact on whether you will experience ghosting or not, experiment with those things until you get rid of it and remember that there are different kinds of light too. Also lowering the 3D setting helps a little.

Ghosting happens when light escapes through the parallax barrier that occurs when your 3DS is displaying images where there is a big difference between blacks and whites.
For example a white character standing in front of a black background can cause ghosting while a blue character in front of a black background won't give you ghosting at all.

It's a really rare occurrence to experience ghosting when playing in normal lightning conditions and it really depends on the software, LoZ:OoT didn't have any at all but I've seen it in eShop demo's before.
Some games are optimized against ghosting like Kid Icarus and completely gets rid of it thanks to in game lightning methods.

Ive always wondered if that actually did calibrate the 3D on the system or was just a measure for your 'personal' level of 3D.

Personally I have no idea. However measuring your level of 3D can be done anywhere, including the home screen.

In fact I find that the home screen is the best place to measure the 3D.

Yeah i didnt have any problems with Zelda or Mario 3D land just this title i play in a pretty normal light room no sun, the "ghosting" was especially bad on The Grid (Tron world.)

That's odd. I had the ghosting issue on only those titles. Every other 3DS game I own I can get in a sweet spot, but not Zelda or my previously owned Mario 3D Land...
