Hacking Unreadable Mirco SDHC memory card

Hey there,
i currently have a 4gb micro sdhc card to use with nds. I also have a usb t-flash card reader for it and all was well..but just recently i am unable to access the data on it via usb on my pc (or any other pc) and get a message either sayin to insert disk (using xp) or to format (using vista) but unable to format. The card is ok as it still works in the nds and the usb works with another micro card inserted. The file system on the card is now apparently RAW and I have searched this on how to format or fix the file system for like ages but nothing seems to help.
Just wondering if anyone had a quick simple guaranteed way (besides smashing the thing).

First try the Panasonic formatter. If that doesn't work, follow Kittymat's advice and format it in a camera (or cell phone).

DO NOT format your microSD card using the Windows format command (which doesn't conform to the SD standard) but DO ALWAYS use the "safely remove hardware" feature of Windows before removing the microSD card from the PC or the microSD card WILL get corrupted.
