Update Neogamma toR8 Final or R9 Beta11 and run it with that.
On any other loader, go to the settings and enable 002 fix (or block 002, whatever it's called).

USB loaders like GX and CFG can load from FAT32 Drives.

well, i did enable the 002 error fix on softchip loader rev99 and tried neogamma r9 beta 11 which strangely had no "fix 002 error" option but i guess it's auto enabled.. but anyway none of them worked! maybe i should use some kind of new ios or cios file? download link pl0x?

That's not the problem of 002 Fix error settings. It might be your cIOS is old.
I had the same problem with my 4.1U, then updated to latest cIOS and 4.2U, now Trauma Team working.
Guys who left insulting comments above are idiots.

very few games are 4.37gb once they have been converted to .wbfs files.

Any that are over 4gb can be split so that they work fine on FAT32 drives.

4.1u, cIOS rev14 (which I promptly updated afterwards, had lent my wii for a few months), Configurable USB Loader, 002 fix

Just dumped the disc to USB and played it no problems. Was quite relieved how easy it went, I had WiiScrubber ready and everything.

Configurable USB loader has fantastic compatibility and is very stable. Its the only loader i ever use, i used to use GX but it got far too buggy.
