Hacking System not turned off properly loop

I had my ps3 all set up, i opened irisman and it just gave me a black screen, so i was forced to unplug my console to turn it off, after that i was greeted with the "system was not turned off properly" message. I pressed X to continue but then the screen gos black for a second and goes back to the same message, tried again and it happened over and over.
At this point incould still access the XMB as well as games and homebrew but i would always be greeted by the same screen, so i formatted the hard drive, reinstalled hen but this time got rid of irisman and opted for multiman instead... Same thing happened, multimam worked fine at first but then it just gave me a black screen as well, and the system got corrupted once again.
So is this a common ocurrence? Or is it my hard drive that is corrupted and will need replacement?
