(I HIGHLY Recommend using both @Bl4ckH4nd and my pack together! as it makes this pack look fantastic.
Place the HD Pack first and then the Xbox Button Pack and overwrite any files it tells you to)
Install:
Download the latest Pcsx2 nightly build (v1.7.2404 +).
Extract the folder to the pcsx2 texture folder (...\Pcsx2\textures\SLUS-20772\replacements)
Config > Graphics Settings > Advanced tab:
Load Texture: ON / Async Texture Loading: ON
Controller Screen (Xbox Button Pack and HD Pack)
Main Screen (Xbox button pack and HD pack)
Character Select Screen (Xbox Button Pack and HD Pack)

Surprisingly this pack only had 1 controller file so it's my smallest pack yet, Huge thanks to @Bl4ckH4nd
for the SSX 3 HD Upscales, they look fantastic and it made this alot easier and faster.
