You're not supposed to modify the original dump. You're supposed to copy the files you want to modify, and then keep the final version. Sm4sh explorer does this automaticallySeems like it destroyed my dump of the game.
Initially it had replaced the CSP's of the Mii's with Mario's, and after trying out the "Fix Missing Characters" box it makes the game crash after selecting Smash. (even after removing the modded files)

Open your modded ui_character_db with param editor. Go to entry 81. Change the value of line 0 to 200. This worked for me; the fix missing characters setting never did anything for me (except enable giga bowser and Big Mac on random character selections). Hopefully this helps others
Entry 81 is for the miis. Line 0 is the cosmetic id, which I believe controls the icons used in the character selection screen. 200 must be a hard-coded value for the picture the miis are supposed to use.

Edit 6/17/2016: Version 1.2.0 has been uploaded, adds an option that probably fixes cosmId issues for random as well as series ID editing.
It has a menu option to fix the random's cosmetic id, running it once or twice has been reported as a working solution to the problem making miis/random looking like mario. Not been able to test it myself (barely had the net to post it). Sorry for the scotch tape solutions but I can't do much better atm. All it does is change entry 59 value 0 to 200 and if that has already been done change entry 81 value 0 to 200.What does "probably fixed cosmid issues for random" mean? Does this fix the Mario icon over miis, or is this something else?
So, I have done Smash 4 Modding for quite a bit now. (Even if it was simple things.) But, I am having an issue with trying to get this to work. I have installed Python 2.7 and the 1.1.0 version of the CSS Editor. However, whenever I go to place files into the directories you have listed, the file doesn't appear, and it says the CSS editor has a forbidden extension, and the file does not appear in the folder. I would really appreciate if anyone could help me with this.

This is my CSS Editor. What can it do? It can...
Usage:
- Rearrange the CSS order
- Quickly change the costume number of a character
- Change the cosmetic ID of a character quickly with a easy to use list of names for changing the look of a character on the CSS
- Enable/Disable Characters to take them off and put them on your CSS
- Change the default rules
- Game type
- Stock Count
- Time Limit and Stock Time Limit
- Handicap/SD Penalty/Damage Ratio
- Stage Select Type
- Customs, Pause, Team Attack, Percent Display, Score Display (On/Off)
- "Fixing" Missing Characters, This turns all the dummy slots into characters so you can quickly add things like giga bowser, giga mac, etc.
- Modify special rules. (Added in version 1.1.0)

Put this in the same folder as ui_character_db.bin and global_parameter_menu.bin and run it with python 2.7 installed. Edit what you want then hit save.

Warning: Using the "Fix Missing Characters" option will permanently alter your ui_character_db.bin copy. Side effects such as CPUs getting finals smashes as their character when picking random may happen, so please be careful. If you want to revert this your best option is to extract it with sm4shexplorer again and start over.
Edit 5/28/2016: Version 1.0.1 has been uploaded, download link has been updated. Fixes mii selector looking like mario if you used Fix Missing Characters and now provides an error message if you are missing files in your folder.
