I wanted to make a cia of a Master System romhack I made that would run well on both O3DS and N3DS units.

The way I did it was by using the Ultimate Genesis Forwarder tool and changing the ROMs extension to .bin (from .sms).

Sadly the Forwarder used a newer retroarch build of Gens GX which unsurprisingly performed badly on O3DS in comparison to the ancient 1.3.2 build.

Short version: try changing the ROM file extension to .bin / .gen / .md

Thanks, sounds good enough for me, as I have the new 3ds. But I would like to share the CIAs, so is not good if they run badly on the old. I think I'll stick with Picodrive for my system.
Have you tried with GG Injector? I have a real Game Gear and a pirate cartdrige that holds a lot of Master System games. I think they should be easy to convert, but they might lose resolution.
Also, there's a Master System emulator for GBA, that could be used to make CIAs, but I think is not that good.

I'd imagine M2 would have to have specifically coded some kind of Master Gear mode to allow for SMS ROMs to run properly on the GG emulator - something probably trivial for them to have implemented but unlikely to have been done.

Well, I thought it was worth trying and to my surprise the SMS ROM boots - albeit with the video being glitched / static, pause button unassigned but also with the PSG audio (music and sound effects) sounding great and confirming that the game is actually running underneath in some capacity.

I don't doubt some of the experts over at smspower could convert SMS ROMs (on a case by case basis) so they could be made playable using the GG VC emulator - and that the process may be simpler than the work needed to build those GG2SMS conversions.

I wonder if your GG cart's SMS games have been modified or whether there's something else going on with the cart on a hardware level (similarly to how those Master Gear passthrough convertors worked). It's beyond my understanding but I'm fascinated nonetheless.

As far as I know, Game Gear has the same hardware as Master System, plus more colors and leas resolution. I don't know if the format of the roms is the same, or ir the pause button has the same coding as start on GG. Anyway, they are creating CPS and NEOGEO injects. It would be great to have SMS, MAME and other systems injected.
