I ran a system check so I could update the outdated IOS's on my Wii and I followed the guide instructions and installed all the wads with success. Then, I ran another system check to verify with ModMii that everything is up to date.

Now it tells me I have to install one more wad which I already installed. Is there a problem with my IOS223? What should I do?

ModMii was designed to work with syscheck b13, b15 correctly identifies the base as 37+38 instead of 75, but modmii still expects the syscheck log to label it as base 75. Anyways, basically u don't need to worry about anything, u are up to date
Since syscheck 2.1.0b15 just came out I thought that it would be better for me to use it to check for updates with ModMii, thanks for the info.
