Is there any way to get a log file so I can figure out what's going on with my controller? I've updated to the second version of 2.0 and it still only works for Luigi's Mansion.
 
Is it possible that not having a GC Memory Card causes the "fatal error app loader size is zero!"?

Format your HDD to Fat32 32kb cluster, I had the same problem yesterday. No other file system/cluster size works.

Done, it's the first thing I checked. There's also a game in the tray... Since there is no documentation as to how to troubleshoot the errors, there is not much I can do. Perhaps it is only related to the iso? I am trying to play SSBM.

Try installing clean MIOS and then DM 2.0, few pages ago someone said it helped him. Apart from that you can try CFG USB Loader MOD r35 and choose correct DML version in options. It works fine for me.

Reset/Power off via GC controller key combo
Anyone know this button combination?
L+R+Z+start
wait a minute you can reset a wii with a GAMECUBE CONTROLLER AND POWER IT OFF AS WELL?

AFAIK it just resets the wii, effectively powering off from GC mode and rebooting back to wii mode.
It is R+Z+Start as fix94 said.
Also: once the partition is converted from wbfs to fat, can you merge 2 fat partitions together without losing data??
The shut down combination is R+Z+Dpad down+B
The idea is that reSTARTing uses the start button
and powering DOWN uses a down button and a red button (B)
