I have the feeling that this particular model of Cobra ODE (5.20c) is very dead. I haven't found guides or tutorials for this very easily.

Also check the QSB is soldered correctly. It is a no-brainer, but really noteworthy. If you have EMU-QSB, check your solder work as well.
Holy moly. This one is very tricky. Make sure that your cobra.cfg, looks like this:
Sometimes the boot disc matters, so try various games. Mine worked with Metal Gear Solid 4 and Virtua Fighter 5.
If all of this didn't work, perhaps consider rechecking your QSB soldering. Worst case scenario, buy a new ODE. With a bit of research you may find one for ~30$.

Holy moly. This one is very tricky. Make sure that your cobra.cfg, looks like this:
eject.on_selection=1
manager.type=browser
eject.add_menu=1
If you want to make your own game folder to an ISO, make sure that...manager.type=browser
eject.add_menu=1
- PS3_UPDATE folder is empty
- PARM.SFO is same or below your current PS3 firmware
- EBOOT.BIN is properly resigned to 3.55 and for 'ODE'
