What does it mean if a game has two cia files?
It means it was split due to the the huge file size. You need to install both.
So I download both files, then I install both using fbi and then I would have one game icon?
Yes, unless they have separate title ids
if I just installed one, no game icon would show up right?
Most likely one is the game and the other is an update or DLC. Is one way bigger than the other?
Never heard of split CIAs, I don't think that's a thing.
On the site I was looking at, it was a cia for dragon quest vii, the total file size is 1.4gb and the two cias were 999mb and 400 something mb, so not dlc

That's just because TWLoader is comprised of two parts, the 3DS-mode frontend where you select the games, and the TWL-mode backend (nds-bootstrap) which actually runs them.
The backend has 2 parts.

Highlight both with your unzipping program (winrar) and click extract here. It spits out a single file for me. Look online how to merge cia files, should be a single command of the above doesn't work
