Hmmm ok so the last thing i can think of is the region in the xml may be wrong, is your vita psn account region US also?
you must put same region as psn account in xml or it will fail.
If the region is correct, there is also a few settings in qcma look there and make sure any relevant settings are changed correctly.
Try getting the firmware and xml files from here if you haven't fixed it yet. Hopefully it helps.
