I tried to show you a tip on how to quickly generate these lines using Notepad++ and Microsoft Excel
(should work on LibreOffice / OpenOffice as well).

In Excel, you need to separate the lines to increment the numbers easily.
For the third column
(with the left-right thing), you need to select the both first lines
(like what I did at 1.00) to properly increment them as many times as you want with respecting the left and right order strings. Then copy-paste all your lines in Notepad++.
In Notepad++, when deleting the spaces
(actually it's tab (but not sure), not spaces) with Ctrl+H, don't write a space, but do a copy-paste of that "space"
(like I did at 1.14 and 1.20) otherwise you'll will break the true space between [itemref] and [idref] strings.
After that your lines should be ready to be paste in your opf files.
