This also happens in stock (before and after hack) and make every game unplayable.

Here is Tears of the Kingdom with this problem, gpu is at 99% and game runs at 10-15fps

The "Real Frequency" on your CPU is low. In fact, based on both screenshots, it's effectively at 25% of the target. Got any homebrew or processes intended to adjust the clocks? Have you tried a clean Atmosphere and then adding things one by one to see what could be affecting it?

I tried a clean atmosphere install and, as I mentioned in the post, it even happens in stock firmware

Doing some tests, the reason for all the lag must indeed be the CPU frequency, which always seems to be proportionally below the target (if I lower the overclock to 1224 MHz, the actual frequency drops to 303 MHz for some reason)

Are you using any form of modchip? And as you're image/video show portable mode, what does Status Monitor report when you dock it?
Doesnt' have any chip or hardware modification (As far as I know, it's a friend's Switch and he may have taken it to a technician several times)

There is Status Monitor when I connect it to the official charger (Full overclock)

And without any overclock

The behavior is very similar in dock, but with worse performance (due to resolution, i think)

You may want to ask your friend if they had done anything to it prior to giving/lending it to you. Was it like this since you got it, or is it a recent development?

My mention of a modchip came from finding this Reddit thread from my searching...

She told me she took the console in precisely because of that problem (the switch starts to feel slow and runs games poorly), and they told her it was just a software issue (So they didn't really fix the problem) I think the problem start one year ago

Looking at Hekate Battery Info, i see that battery capacity shows... 7946 mAH? Is this normal?

Wow, is that even possible? I should ask my friend about this. So, can a battery replacement solve this?
It may be that while installing this battery damage has been done. So i am really sceptical this alone will help.

Yeah, that makes sense. Then, I'll take the console to a technician to diagnose the problem and see, if it's a hardware issue. I opened the console but didn't see anything I could fix myself.
