Either remove your main account, or just never log into it after modding your PS3. This can result in a permanent ban and will effect other devices that use the account.
Check to make sure your ps3 is able to be modded with this method by using the minvercheck.pup in my mega link!
Install OFW 4.82 twice to avoid the potential corruption during the CFW process.

I would highly recommend making an alt account to use only on the cfw ps3, and make sure that auto sign-in is off...
Please double check that you have auto sign-in off, I hate to see people getting banned for not knowing certain things. Hopefully this guide will help prevent some bans

1. Format your USB to FAT32 with the GUIFormat.exe that is provided in the rar from the Mega Link.
2. Place the file named flash_482.hex on the root of your USB.
After moving flash_482.hex to your USB make sure it matches this MD5
MD5: d05be52f8d21700052fbd1fc0174acae
3. Make a folder called PS3 and in that folder make a folder named UPDATE.
4. Download the CFW of your choice (In the mega folder link), put the PUP file in the UPDATE folder and rename it to PS3UPDAT.PUP
5. The pkg files you see the the Mega folder link are what I suggest to install after modding your ps3. (we will get into this later and I would just wait to place them on your usb until afterwards)

We are going to be using ps3xploit.com for this guide(Not sure If I can post this link )

1.Open the Browser on the PS3 and type in ps3xploit.com, choose the exploit you need, and set it as your homepage.
DON'T start the exploit just yet!!
2. Close your browser, reopen it, and then Select the path that applies to you and press [Exploit Initialization]
and wait until it succeeds. If it fails, it should have instructions to refresh the page. (This could take a few trys)
3. Launch the nukes! (Kidding) Hit Patch and wait for success!
Extra Steps- I recommend checking the flash memory for errors before restarting your PS3. You can skip this if you REALLY REALLY don't want to do this or feel that its safe enough.
1. Load ps3xploit.com and load the flash dumper.
2. Dump the flash memory and check it with this- https://github.com/Swizzy/PS3DumpChecker

So this is where you decide whether you install Ferrox or Rebug (or another CFW or your choice? I miss Rogero , Please come back Rogero )

(Settings) >
(System Update), and select [Update via media]
If you are going with Rebug, you can either install 4.81.2 REX or 4.82.1 REX LITE.
If 4.81.2, Install Ferrox and go to the Next Tab
If 4.82.1 LITE, just follow the same steps as Ferrox

You're gonna need to install Ferrox regardless if you want Rebug 4.81.2 Rex
So install Ferrox 4.82 and download the Habib-QA_Toggle-4.21+(standalone).pkg and enable QA Flags
If that doesn't work, try Habib-QA_Toggle-4.21+(standalone)v1.02.02.pkg
Run the QA Toggle, the system should reboot and then you can install Rebug 4.81.2 without any issues!
If it does not reboot on its own, (Which it should) just reboot and everything should be good to go!
After the reboot make sure that you can open up the debug settings to make sure that QA Flag is enabled.

Follow all of this and you will have a slimmer chance of getting banned!
I say slimmer instead of a 100% chance of avoiding ban because I'm not exactly sure what is causing the bans.

Open PSNinja and Select the top 3 items.
Wipe History, Replace History, and then Make History Read Only
After doing this once you shouldn't need to do this ever again, but I did it on every boot anyways by making it writable and redoing the 3 top selections.
As tedious as it may be, I never got banned and remain unbanned.
PSNPatch should be quite self explanatory, but here.
I haven't updated this homebrew app in quite a while and it seems that they have added quite a bit!
I personally would install the PSNPatch plugin so that it blocks logging into PSN until Syscalls are disabled.
What you want to do is Select Disable CFW Syscalls. This is the closest thing to OFW that you can achieve. You will need to do this on every boot to make sure that you avoid getting banned as much as possible.
This essentially emulates OFW and disables anything that CFW has access to for that boot.

Avoid running homebrew online! Even if its stealth! IDC what people say, avoid this at all costs.
If you're Blu-ray Drive works and you use Multiman, before disabling syscalls, make sure BD-emulator is enabled in Multiman and Load whatever game you are planning to play. This will emulate the game as if it was in your BD.
Avoid cheating online 100%!
As much as I did it back in the early days (Yes I know, judge away!) It's just a one way ticket to either a game ban or a permanent PSN ban.
