Well, you should never hack with videos on youtube. They are old. I suggest your hack your wii with modmii. Its stickied on the homebrew and hacking forum. Also instead of usb loader gx use CFG.

Your cios 249 and 250 are outdated use modmii to get d2x cios if you want the newest d2x cios look for beta in cios section. Install with the wad manager of your choice.

You should install CIOS 222, than you are sure both usb ports work .
Use the homebrew app called hermes installer, remove gamecube accessoires and choose to install.

Your IOS is idd outdated.

You have a weird setup. Your cIOS249 and 250 seem to be based on the same IOS. Normally you would have 249 with base 56 and 250 with base 57.
This doesn't explain why your flash drive doesn't work, but it's a good idea to update them with ModMii anyway.
Make sure you are using the bottom USB port on the Wii. On most cIOSes (except d2x v10 beta 53) that's the only port that will work and is always the recommended one. Have it inserted in that port before you start the USB loader and it should work, whether it's WBFS, FAT32 or NTFS.
