Russia enabled North Korea, so Iran would be no exception, so they have miliary alliance.

Iran has been supplying Russia with drones for their invasion of Ukraine. It's hard to predict how Russia will react if and when the Iranian government reaches the verge of collapse.

Iran has a population of 92 million, so they can throw tons of meat into the grinder to get their revenge. On top of that, It's the US and Israel with no other support. Trump has done enough to alienate the US completely from all their allies. The US could get nuked tomorrow by Iran with a Russian nuke and the entire UN would go "Well, they had it coming."

I do think they will target Israel first to send a message to the US and it is potential to expand the nuclear conflict to wider region, so it would be tough for the US to make a decision and avoid further conflict, they could force Israel to end the military campaign to deescalate the situation, also Iran could be penalized with full sanctions and become pariah to most world. If Russia help them, so they will be pariah too. If China support or enable will be pariah too.
