Okay. Thanks anyways
I extracted the cia, did some modifications here and there and injected a sega cd game in bin/cue format (Keio Flying Squadron) along with the bios and worked like a charm (audio included).
It is a more advanced way to do it but it can be done.

Has it been updated? I don't remember the program accepting the CD Iso files, though I do know the retroarch emulator it uses does support some CD games and romhacks like Sonic Megamixv4.

So, how exactly would I do this? I'm completely clueless when it comes to this kind of stuff?
I created a cia with the ultimate maker using another game (I used Ristar in this case but any other do) but put the name and banner of the sega cd game I wanted to inject (Sonic CD in this case).

Then used this to extract the resulting cia.
Once I had it extracted, took a look at code.bin inside the exefs and found this

so I changed it to load rom.cue instead of rom.bin

Then added the cue and bin of the sega cd game to the romfs (japanese sega cd bios and Sonic CD in this case)
upload_2017-5-20_14-50-42.png

renamed the cue file from whatever the name it was to rom.cue (the same as the edited code.bin)

And last but not less changed the retroarch system directory to romfs: in the config file (so retroarch search for the sega cd bios there)

Everything done I repacked the cia, installed and in a couple minutes was playing some japanese Sonic CD

The ultimate forwarder maker hasn't been updated, I just used it to create a "false cia" with another game and then inject the sega cd bios, cue and bin files.

I have installed the sega cd cias in both, an old and new 3ds and the old 3ds is too weak to run sega cd games at full speed, only the new 3ds is capable of running them at a playable speed.

So far I have tested:
Keio Flying Squadron
Sonic the Hedgehog CD
Popful Mail
Earthworm Jim Special Edition
Batman Returns
Megarace

All of them run perfectly. I haven't tested any sega cd 32x game.

@Sakitoshi Which emulator core did you use when making your Sega CD injects? I'd like to make a Sonic CD forwarder when I have the time.
First of all, this only runs well on New 3DS, but only if you use picodrive.
Gen Plus GX running Sega CD is slow even on New 3DS, picodrive runs just fine and without any noticeable slowdown.

N3DS? Check. Picodrive? Check. Thanks!

Edit: Ok, @Sakitoshi I made one CIA and it's hanging at the 3DS bootlogo. Did you turn on the rewind feature? I gave it (and the auto-savestate) feature a try, and I'm not sure if it's part of the problem.

It could also be because I tried injecting the hack Sonic CD ++. As far as I know, it only comes in .iso format, so I made a .cue file that references the .iso and the .mp3 files, but I'm hearing that Picodrive for Retroaech doesn't support .mp3 files, so maybe that's the problem.

Picture is distorted.
Game: Contra: Hard Corps. Settings: GX core, bilinear filter off, 320x244.
Just look at the Konami logo at the start of the game with these settings. Logo looks fine with RetroArch on PC.

Can this handle Sega CD? I would like to play Sonic CD!
You're probably too late anyway, but here are some quotes for reference:

It is a more advanced way to do it but it can be done.
