Install Devmenu and look in the programs area for one that is installed to the ctr nand. Mkae sure not to uninstall anything that is read. But this happened to me after updating my cfw files and 2 of these icons had showed up. Lucky for me, they were both FBI.cia's installed onto my sysnands nand on accident and they transferred to my emunand so I just found the ones i needed to uninstall and did. If you do find one, search the id of the cia before deleting it though. I found this out by searching reddit :P Not taking credit

I fixed this by going into DevMenu and finding that I installed TWLSaveTool to CTRNand twice and to SD once, I deleted all 3 accounts of it and reinstalled it! Black icon GONE! (at least for me, find out if you made a mistake long ago)

10.7.0-32E now, but it started from 9.2.0-20E
I have my emuNAND on a microSD in a SD adapter, and yes I got many of those.

Thanks to HANS I know that the package named "???" ID is 0004001000022A00, and it is a 3DS update file so that's not the culprit. I also delated all updates and extradata from games I don't play anymore and, still no luck:(.
ok so if its an update file then (you likely want to keep it)
use 3dnus to download the update (packed as cias of course) and put the 0004001000022A00.cia you get from 3dnus onto your sd card
then (on your 3ds) use fbi to delete the installed cia and reinstall it (emunand destination nand). . . this should solve your problem

fyi if you clone your sd card to a second sd card first you can test this without any fears

IIRC updates are also per console signed, so one needs to download it again via the eshop.
One time installing a backup of my dlcs with all characters cleared and uninstalling and reinstalling via eshop wasn't successful and I had an older version of my ticket.db and had the eshop reimport my bought tickets into it.
Don't know a solution for your case though.

Thanks for trying to help me but I removed everything With FBI in my emuNAND HOMEMENU, but the present was still there so it's something in the emuNAND Firmware and FBI can only delete files from NAND, so I'll just have to live with that for now, thanks again.

umm your nand is the where the system firmware is stored and emunand is a 1 to 1 copy of your nand so your statement does not make sense (if fbi can read your nand then it can read a 1 to 1 copy of your nand as well) . . . and also makes me think maybe you dont fully know how to use fbi

load up emunand
in fbi if you press "R" 2x and "L" 1x now you are looking at emunand destination nand in delete mode
carefully select the ??? app that you identified as 0004001000022A00 delete it . . .
then from there press "R" 1x now you are at emunand destination nand install mode
install the 0004001000022A00.cia you downloaded via 3dnus

if you did everything correctly (and the ??? title really is 0004001000022A00 . . . as you say it is) then this should . . . will work

Install devmenu, go to programs, and scroll down until it either has no id or is installed in the ctr nand and isnt a system app... it wont show titles other than the browser and a few others (the red titles). I believe ive said this in this thread already

Well it didn't work and now I'm sure that the 0004001000022A00.cia is not at fault, cause when I deleted it and went to the homemenu the black icon was still there, but the box with "???" name in homebrew was gone. I don't want to check every 138 Titles in my emuNAND, is there a way to display the software ID's in rxTools by selecting a icon in the homemenu, if there is please someone tell me. If not I will end the treat, not worth it

Nothing suspicious there.
