Hardware PS5 stuck in a safe mode boot loop

I have a PS5 EDM-020 that's boots straight into safe mode and only gives the option to connect controller via usb. When the PS button is pressed it proceeds to "please wait" then reboots again to the "Connect the controller using the USB cable and then press the PS button."

No other options are available, pressing and holding the power button until a second beep is heard still brings up the connect controller via usb screen.

UART finds no error codes.

Any ideas?

TRY different controller or ports?
 
The controller is being recognised and when the PS button is pressed, it then goes to the "please wait" screen and stays here for several seconds before it reboots back to the "Connect the controller using the USB cable and then press the PS button."

I don't get any opions at all like there should be in safe mode.

Where are the safe mode options loaded from? The SSD or the BIOS?

could the SSD controller be at fault or the BIOS?
TRY different controller or ports?
