Normally is by two factors:

Tomato sauce or Tomato Paste is a smoothie, you are right but most of Ketchup HAVE NO TOMATO ON IT, by example the most famous «Ketchup» on my country is «Salsa de Tomate Fruco» (now is unilever) and if you read the ingredients is made of pumpkin smoothie, tomato powder and tomato flavor but have no real tomatoes on it. The original Ketchup is a fish sauce.

I don't know about where you are from, but here in Canada, there is a minimum of natural ingredients that are required by law.
For exemple, cheez whiz, here, is made from actual cheddar (like 50% cheddar or something). Canada's law are more strict about artificial food and you can't just make your food 100% artificial.

Here on Colombia that law was revoked some years ago, mainly because some «traditional» products are called on a way but made from other things.

The most outrageous is our traditional liquor called «Aguardiente»(schnapps) but really is a «anisado»(anise liquor) and have nothing to do with a Aguardiente. but for tradition is called that way.

but what is now mandatory is the ingredients HAVE TO BE ON THE BACK OF PACKAGE IN READABLE FONT.

I never really gave it a thought. I do not know if the connecting bread affects the nomenclature.

If you were to use the term "Hot Dog sandwich" around me, I don't believe I'd object.

It's just cultural faux pas at worst. In my part of AmericaTown, they wouldn't appreciate the addition of unnecessary words.

In Spanish do not affect the nomenclature, because the word for sandwich is «emparedado» which literally means warp in bread. I think in English is something similar.
