I would want the middle mouse button on my mouse to act as the start button.
Is it possible with NO$GBA?
A Google Search returned no results, so I'm asking here.

The purpose of being able to play TWEwY with only a mouse.

You can use Xmouse to map your middle button to a keyboard button that you use as start. Pretty handy, because you can also bind keys to other buttons. http://www.highrez.co.uk/downloads/XMouseButtonControl.htm
