How do I unlock that? I've beaten the entire game but haven't collected all the coins. Could that be the reason? Has anyone else unlocked it?

The only way I know is through a cheat code.
Go to the unloackbles menu, hold down L and R and press A, B, A, B, A, A, B.

Yeah, when you get it, go to Single Player -> Continue, point your cursor on episode 0, then press UP.

I would think when you complete the first 45 episodes, credits will roll and at the end it tells you the code. I skip credits, though.
