I noticed when I was in GAME the other day that "My Health Coach" is now released in Europe, but it's not present in the Release Information list. When do games get put into this list, and who allocates the release numbers that go with them?

Games get added to the list when they get dumped and uploaded to the internet.
The release numbers are just one more than the previously uploaded game.
Usually it's the uploading group that puts the number, but sometimes two different groups upload two different games with the same number at the same time, so it's a mix-up.
