Mainly iTunes, but Songbird if I'm on Linux. I would install iTunes on Linux, but WINE is a bitch.
 
I use 'Media Player Classic' for virtually all my Video/Music on PC (small/compact player - unlike Windows own)

On mobile (Nokia5800 - that's MY MP3 player) I prefer to use 'LCG Jukebox' for music (Don't like the way you have to go through Nokia's own music player via 'Categories'>'Playlists'>'Now Playing' just to listen to music)
 
I was introduced to Media Player Classic by Fast and I haven't looked back since. I don't really play tunes on my PC so don't use any music players.

iTunes, Quicktime. I know they run sluggish on PC's, but its actually quite speedy on Macs.
I also use my portable media players (iPod Classic, iPod Touch, DS, PSP, etc)

VLC for those rare weird formats.
It's really useful!
 
for (bluray) movies I use my PS3

I really like the WMP11 for Music (since it can be put into the toolbar of vista and is always visible and real-time search so no need for making any libraries) and VLC for video (since it plays almost everything).
 
Windows Media Player for music.
VLC media player for AVI video files.
Media Player Classic for MKV video files.

I usually only watch AVIs and MKVs, thus the reason why I chose those two. (:
