I was more thinking about the
priiloader UNEO Booter, or
bootmii uneo booter. Also, I'm not sure your included channel has the code to read the arguments from meta, and check the loader's settings and dol the same way UNEO does.
Though, the official forwarder seems to have the same init issue than the loader, even though it uses IOS58.
Wiiflow can init all drives properly, but USBGX seems to have issues. maybe your channel does that initialization better. I didn't try.
Don't change the titleID, it would overwrite and conflict with the channel. some users might not like that.
maybe an option to choose which channel to install is better? users could install the uneo forwarder directly with modmii if they choose to install that USBloader. there are two different channels on the repository: wii and vwii.

when we help someone, we expect the user to follow what we suggest, do it in the proper order, and report back or have a communication before attempting anything else, not trying things randomly in hope it's fine. if you need help, it's because you already didn't do something correctly, listen to people who takes time to help you "in future".
if you still want to fix your console : you miss cIOS in slot 249. that's your problem. the loader can't init the drives if there's no IOS to use.
though, if you had the latest version of the loader, it should also try IOS58 automatically, so if it didn't work you either have an old version of the loader (not surprising) or your device is just not compatible with that IOS (I doubt it), or your USB port doesn't work anymore on your console. There are different ways to test all these possibilities. if you come back here...
