Hacking Beginners Guide to Making Themes on 4.1U

I am currently on 4.1 NTSC with the darkwii extended theme. I found this cool health screen someone made that i want to put on mine. Unfortunately, I cannot find any guides that will help me with a 4.1U wii. The only guides I find are for a 3.2 and 4.0. So, I am wondering what I should do. Is there anyone who knows how to make the image below into one that will work for a 4.1U or can someone point me into the direction of a guide that can show me how. Any help would be great. I am sure there are others out there who are in the same problem as me.

Also I do not have healthii or softmii. I am assuming you are able to install this on the wii using mymenufy. Am I right or wrong on that.

I was also curious if these themes have the press "A" button like on the normal health screen.
