How are you performing the installation? Are you following the on screen prompts correctly?
Try changing the theme again, to default. Then When loading menuhax, try the uninstall option first before installing it.

IIRC, the luma folder is generated automatically when you launch Luma3DS for the first time.He may updated the guide but not all parts.

"Copy the Luma3DS.dat file, the 3ds folder, and the luma folder from the Luma3DS zip to the root of your SD card"
but on luma3ds.zip there is luma folder

Starting with version 6.0 luma folder is created when first loading luma CFW, that is why is no longer included in the archive. The config file is inside, after saving the configuration.IIRC, the luma folder is generated automatically when you launch Luma3DS for the first time.
