Im working on a idiot proof guide for all my internet friends with Wii's!
I have only tested this on my own Wii and will not be responsible for any damage done. The following steps have been repeatedly done on my own Wii without any problems!
This is NOT for LU64+, but with some modifications (feel free to cut in the text and post modifications) should work!
Things like not downgrading etc, but I don't have such a Wii and thus not able to test anything!

This tutorial will guide you STEP-by-STEP into installing 4.0 with truncha patched IOS'es onto your Wii.
This will NOT work for LU64+ Wii's, I sadly do not have such a Wii and can NOT test anything!

Here is a quick summary of terms used:
"slutty" A Wii with already truncha patched IOS'es.
"virgin" A Wii without any trunch patched IOS'es.
HBC The Homebrew Channel.
Wii Options The little Wii bubble in the bottom left corner.
Home The home button on your Wii-mote, between - and + ...
And to be VERY clear, I make a few assumptions:
A) You RTFM! (Read the Fucking Manual)
B) You RTFS! (Read the Fucking Screen)
First copy the contents of "SD" folder to a the root of a (formatted) SD card.
For Preloader you need a hacks.ini file in the root of your SD card.
The latest Preloader's hacks for you region are here: http://wiibrew.org/wiki/Preloader/hacks
Just copy and paste the hacks and save it to "hacks.ini" on your SD card ... ofcourse the 4.0 hacks for your region!
I have made two files, you can just copy and rename.
Put the SD card in your Wii.
On a "slutty" Wii: (Skip this if your Wii has no truncha patched IOS'es!)
Goto Wii Options > Data Management > Channels > SD
Install HBC.
Install BootMii (as boot2 if possible AND as IOS).
Exit installer.
If you DO NOT have a BootMii/boot2 install AND no GameCube controller:
!!!I have not refined a retard proof method for converting NAND images from other tools yet.
!!!All NAND dumpers I tested sofar do not dump usable images for use in BootMii's RestoreMii.
!!!And due to a bug in the BootMii/IOS it does not (currently beta2) allow you to use the power/reset/eject buttons.
!!!My advice is to get a GameCube controller before you continue and make a NAND backup with BootMii!
If you DO NOT have a BootMii/boot2 install, but a BootMii/IOS install AND a GameCube controller:
Press the home button and start BootMii.
Select the 4th option and then the first.
Follow the instructions on screen to backup your NAND!
Start HBC in the main menu of BootMii.
If you have a BootMii/boot2 install, you will need to use power to switch choices and reset to select:
Turn your Wii off and on, it will show BootMii.
Select the 4th option and then the first.
Follow the instructions on screen to backup your NAND!
Start HBC in the main menu of BootMii.
Start HBC (or should be already started).
Run "NANDClean".
Press A 10 times, please look at the screen and only press A when the program asks for it!
Run "AnyTitle Deleter".
Goto "00000001 - System Titles" and remove all IOS >200.
Press home to leave "AnyTitle Deleter".
Start "AnyRegionChanger".
!!!If you have NO working BootMii/boot2 install the following step COULD BRICK your Wii!!!
!!!This is because IF Preloader was to be installed first, "AnyRegionChanger" would simply remove it!!!
!!!But if you RTFS everything should be fine, the settings showed are your current settings!!!
Double check if the settings are correct (should be by default).
Goto "Install System Menu 3.2" and press A and select yes.
(THIS MAY TAKE A WHILE)
After installing System Menu 3.2 (atleast on a 4.0 Wii) the warning appears on the top of your screen, tripple check your settings and if they are correct ignore the warning.
Select "Reboot to System Menu" when its done and your system should be at 3.2 now.
Start the first option in BootMii (System Menu channel ... Wii icon).
Goto Wii Options > Wii Settings > Wii System Update
Your Wii should restart and is now a "virgin" at 4.0!
On a "virgin" Wii:
Goto Wii Options > Wii Settings > Check the top right corner if you are at 4.0.
If your version is not 4.0 goto Wii System Update and let it update your Wii to 4.0.
Go back to System Menu, either via the home button or by going back manually.
Wii Options > Data Management > Channels > SD
Install HBC.
Install DVDX.
Install BootMii (as Boot2 if possible AND as IOS).
Exit installer.
If you already made a backup in the "slutty" steps above you can skip the following NAND backup steps:
If you DO NOT have a BootMii/boot2 install AND no GameCube controller:

On the SD card rename the "bootmii" folder to "bootmii-orig".
Copy boot.dol from the cBoot2 folder to the root of your SD card.
Put the SD card in your Wii.
Start HBC (or should be already started).
Press the home button and pick "Reload".
Start cBoot2 Installer it will run and then exit.
Press the home button (while in HBC).
Start BootMii and it will start cBoot2, which will start WAD Manager).
Install cIOS249_rev9.wad.
Press the home button twice after installing.
Start HBC.
Start "cIOS38_rev13b-Installer", leave IOS at IOS249 and press alot of A to install ... RTFS!
Start "IOS Downgrader".
Press A and wait a while for it to finish.
When its done press any button, Wii will be restarted to System Menu.
Start HBC.
Start "IOS60-61_Shop_Mii_Installer", press alot of A (RTFS) to let it do its business and exit.
Start "Preloader".
Press 1 twice to get past the warning screen, it will install and exit to Preloader.
Goto "Systemmenu Hacks" and select the following hacks:
- Replace healthwarning with backmenu.
- No health check.
- Skip disc channel update check.
NOTE: The region free hacks do not work for all titles, some just don't work with them turned on and SoftChip/NeoGamma should do this correctly.
Select save settings and press A, press B after the settings are saved.
Goto "Settings".
Set autoboot to "Systemmenu".
Set return to "Systemmenu".
Set "Ignore standby setting" on.
Select save settings and press A, press B after the settings are saved.
Select "Homebrew Channel" and press A to start it.
Put SD in PC.
On the SD card rename the "bootmii" folder to "bootmii-cBoot2" and "bootmii-orig" to "bootmii".
Rename "boot.dol" to "boot.dol-cBoot2".
Open up the file "bootmii.ini" in the "bootmii" folder with WordPad (as Notepad doesn't show correct line endings).
Change "#AUTOBOOT=SYSMENU" to "AUTOBOOT=SYSMENU", remove the "#" this is called uncommenting.
Put SD in Wii.
Your Wii is now at 4.0 and running with truncha patched IOS'es, congratulations!
Now its time to install some WAD's!
First press the home button and pick "Reload".
Start "Wad-Manager_v1.4".
Press A, leave IOS at IOS249, press A, press A.
Install the following WAD's:
NeoGamma R4-NGBL.wad
SoftChipR99.wad
USB Loader GX-UNEO_Forwarder.wad
Install the following to get a "WAD Manager" channel.
WAD-Manager_v1.4.wad
Press home when done to get back to HBC.
