I had the same problem.

Did you install a Cios before?

How to fix it:

First try the Cios Uninstaller V1.1. If he doesnt find any Cios no problem.

Then try the Cios Uninstaller v1. It detects a Cios for me and uninstalled it.

Thats how i fixed it for me.

Cios Uninstaller V1.

Your an angel v1 detected the messed up cios 249 and removed it and I was then able to correct all the problems and start fresh

I don't know if anyone can help me, but I thought i'd ask. I just got the wii for christmas and decided to mod it to play backup games. I updated it to 3.4 firmware before attempting the twighlight hack. I got the homebrew channel on through using the tp hack. When I tried to put on the iso36_rev7, I got an error -2011. Which of course led to the same error trying to install the dvdx. I've since downgraded to 3.3 and 3.2 trying to see if that helps to no avail. I've also tried to use AnyTitleDelete (to delete iso249) and I get error code -1017 starting out gate. I then tried to use ciso_Uninstall, but can't seem to get that to even show up in HBC. I've thought of doing a full reset (can I do that?) and starting over again with TP hack, but thought i'd see if yinz had any ideas at all. Any help is greatly appreciated.

What i did was use Cios Uninstaller v1. through the twighlight hack all I did was place the cIOS-Uninstaller.dol in the root of the memory card and renamed it to boot.dol and then ran the twighlight hack and the iso249 is detected and can be deleted

That is exactly what I am doing:
Thats why I wrote:
I tried the 'IOS16-64-v257-prod.wad in wad manager 1.3- it installs.I press home-wii restarts.
Then I go to custom_vm folder and it opens wad manager 1.3-, thats what it says on top of the screen, and it says IOS16 detedcted.
Should the title on top be different? and not wad manager 1.3- both wad managers look the same to me. but the Custom VM says ios16 detected.

1. Use cIOS uninstaller to remove all custom IOS'es and start from scratch.
2. Install ios16 using wad manager 1.3
3. Install cios_fix using custom WM
