Hardware SD card problem

I have a toshiba 8 gb sd card. I use it on my wii. It perfectly runs on my wii but when I plug it to my PC, it randomly disconnects (usually when entering to a folder or making changes.) then it reconnects. I ran scanner on windows, it told me that it fixed the problem but it didnt. Can you tell me why this happening and a way to fix it?

do you have a diffrent sd card reader you can try?

It's a reader issue, not a card issue. Every time I get a memory card reader, it fucks over any card I use with it. The windows scan only fixes issues relating to the card's file system, not the card itself. I'd say try to find a better reader.
