I assume you are after hacks rather than translations which are covered elsewhere.

http://www.jeffludwig.com/dq5ds/download.php and the fire emblem sync patch ( http://filetrip.net/nds-downloads/r...mblem-shadow-dragon-sync-patch-10-f27502.html ) are worth a go.

If you are going to exclude pokemon, NSMB, mario kart and fire emblem you will likely not be left with much (there are good hacks among those franchises just mentioned but you will have to wade through a lot of less than stellar hacks first). There has been quite a bit of non translation work going on but it has yet to really be turned towards making improvement hacks like you might see on the earlier consoles or even the GBA -- you will tend to mainly see content rips, language mods, always applied cheats and a lot of sound hacks rather than new levels, level mods, rebalancing hacks, spoofs or total conversion type hacks.
