Insert it into a card reader, not a PSP. Format. If you can't, the memory stick is dead.

You didn't softmod the battery yourself did you, but bought it already Pandora'd? Well it would seem that:

a) Your Slim isn't capable of EEPROM writing and so cannot softmod/unsoftmod a battery.
b) Your Sandisk MS doesn't have a multiboot IPL on it.

Format the stick and then use Rain's IPL & Memory Stick Tool 2.0 to inject the TM IPL.
