I was trying to give DSReader an icon and name so it wouldn't show that default icon and NDS Program File or whatever default name it gives....

but when I try to edit these things in DSBuff...

1. I put an icon, of 32x32 size and 16 colors, as the DSReader icon. It's a book, obviously.
2. I change the displayed name to DSReader and the creator's name and whatnot.
3. I click save. It glitches the icon.
4. I fix the icon and click save.
5. DSBuff gets an error and the .nds file has to be replaced because it's been broken.

Can anyone send me a premade DSReader .nds file that has the name and icon fixed for Wood R4?

I think the problem you're having is that you're not ticking the Homebrew box when you save the ROM.

I've done everything.

I've saved without the box ticked, I've saved with it ticked... Well, actually, that's a lie.
With the box ticked, it perpetually tells me I need a 32x32 icon with a max of 16 colors.... Even if the icon I picked has exactly those specifications. So it won't save with the homebrew box ticked.

Make sure you're using a clean copy of the game/homebrew, not one you've tried to edit before.
Then make sure your image is 16 colors, not 16 bit color. Windows Vista/7 Paint can do 16 color BMPs.
I'm not sure about GIMP since I don't use it, but in Photoshop it's fairly easy but not obvious:
1). File -> Save for Web & Devices... (Alt+Shift+Ctrl+S)
2). Set it to GIF (Default) or PNG-8, and select 16 Colors from the drop down.

Only other things I can suggest are:
Make sure you're using DSBuff v1 Final.
Leave the Internal Title with the default info ("." with 9 spaces).
And tick Homebrew box if it's a Homebrew game.

I haven't had a problem making/editing icons.

I haven't had a problem making/editing icons.
1. It was 16 colors. I indexed it to be so. 2. It was the right size. 3. I was using the latest DSBuff. 4. I left the internal title alone. 5. Ticking the homebrew box either made it perpetually say I needed a 32x32 icon with 16 colors... or it would get an error and need to be restarted.

Either way, it didn't work. I redownloaded DSReader like twelve times.

Thanks for the help though.

If you want to upload the image (in a zip maybe, to keep any image host from potentially altering it), I could take a look at it.

If not, I hope you have better luck next time.

I tried to recreate the problem but now nothing's happening.
