Are you trying to run "Add Games" or a game?
I cleared my TTMenu folders, and added games to TWLoader. However, I have some emulators on my home menu (just so I don't have to go to a different place for emulators,) and those won't run. They (instead) will load the last game I loaded in TWLoader...

What's in "sdmc:/_nds/YSMenu.ini"?
 
Oh, sorry XD. I kinda forgot Micro SD's couldn't be locked XD. No, it isn't.
Well then, dunno why it isn't working. If your flashcard is set to DSTT/R4i-SDHC/etc., then the YSMenu.ini file in the SD Card should've changed on game launch.

Weird, will it remake the YSMenu.ini file if you re-select the flashcard?

Unless of course the INI has broken permissions or something. This isn't likely, but all possibilities should be covered. Note that the 3DS follows the UNIX standard for file permissions, so Windows (NTFS) permissions don't carry over. This causes problems when transferring files over something like FTP or the mSD management app (which uses a bastardized version of Samba that barely meets the criteria for UNIX standards).
