IOS249: 249d2x56 v6 (base 56, obviously) - This works with 99% of games. I highly recommend it.
IOS250: 250d2x57 v6 (base 57, obviously) - This works with games like Black Ops that do not work with 249d2x56 (because some games require IOS57). I suppose you could also use IOS224 instead.

I am not entirely sure what you are asking. If you are using a forwarder channel on your Wii Menu to launch, say, Configurable USB Loader, then it is just forwarding you to the application on your SD card. It doesn't really matter which one you use. Whichever is most convenient for you, I suppose.
Where did you get the 222 v5.1 from? 222 is supposed is recommended as v4.
@KoG: Forwarder ISO's are fake games that just load channels (both on the Wii and emulated NAND) and homebrew. They require base 38. Just about any homebrew that uses 249 requires base 38.
If you only plan on playing discs, go ahead and use base 56 for 249. If you want to expand past that to more complicated things, install base 56 to 248 and make that your default for USB loading, and put base 38 in 249.
You can always change it later, so for now you should probably stick with base 56.
