I have lineage os up and running, ableit very very sluggish with an a2 card even (menus and getting around is painful, but titles seem to run ok).

With games installed, how is everyone switching user profiles per game or per android?

I tried adding a second android user, but the google play apps installed under user1 dont show up, even though user2 is a family library member of user1?

Any thoughts?

You can only share system applications. You can use the Gapps package and install it by Recovery, they would be installed as system applications that can be shared with multiple users, and they would not be removed if the system was restored.

Another alternative would be to use an application that converts the non-pre-installed applications into system applications, but root is required, so you should also install the Magisk package by Recovery.

Each user has its own apps on android

Thats a major bummer, means for me about 3-4x the space needed.

The goal here was to let the kids/wife play the games under their "google play" gaming accounts, not just using mine, seems a bit much effort to achieve that i guess.

It s good for privacy, each profile has a different id
