When using 4.2 firmware make sure you aren't trying to run bannerbomb from "Settings, Data Management, Channels, SD Card", you must run it from the SD Card menu (click the SD card icon on the main system menu).

Problem solved with following error:
Ran hic_03.zip in virtual machine (thanks for the suggestion, blitzxp) to create hacks.ini
Preloader .28 must be more picky than .27 with CR/LF as I'm using Ubuntu - verified that there was only LF (no CR) in the file.

Went ahead and installed .27 again - preloader .27 showed hacks...
Installing Preloader multiple times will not mess anything up.
It will detect that there was a previous version and overwrite it with the newer version.
It is a good idea to run the hacks remover wad before reinstalling Preloader.

My LCD TV does handle PAL. Seems that Kororinpa doesn't have NTSC video support because when I forced to NTSC in Gamma it hung at a green screen. When forcing to PAL or not forcing, my resolution was showed to 576i.
