[Help] PS5 Slim crashes to Safe Mode (reinstall system option only)

Hey everyone, I’m in a really frustrating loop with my PS5 Slim and hoping some of the devs or experienced users here can shed light on what I'm doing wrong, or if there's any way out of this without a full wipe.
  • Console: PS5 Slim
  • Firmware: 12.0
  • Exploit: Jailbroken via Gezine's Y2JB backup (YouTube app)
  • BD Drive: Unpaired (I used bdj_unpatch.elf previously, lost pairing during a previous OS reinstall, and can't re-pair without updating FW).
  • The Issue: The console hard crashed and is now stuck in a Safe Mode loop. It only gives me Option 8: "Reset PS5 (Reinstall system software)".
This is the 2nd time this has happened.The first time, I installed a bad game backup directly to the internal drive. It crashed the console, corrupted the drive, and forced a reinstall (which is how I lost my BD drive pairing).

This time, I didn't install any game backups on the internal drive. The only major thing I did prior to this crash was restore my old saves with ps5-bar-tool & Garlic Save Manager

Shortly after this, the console hard crashed and now I'm completely locked out in Safe Mode again.

Any idea what would cause the issue ?
 
The OS got corrupted because of the hard crash so your option is to reinstall firmware and redo everything over.
