I dont know if i should update. i haven't been on in awhile should i update to 11.0. 1?
if so what cfw should i use? thank you guys for the help

There's no reason not to update to 11.0.1 if you aren't using SX OS. If you're one of the minority of people still using SX OS, don't update to anything higher than 11.0.0. If the Switch is ipatched and on 4.1.0, you obviously don't want to update the sysMMC to anything higher than 4.1.0.

Atmosphere is the recommended CFW.

Yes, you should always update to the latest version of Atmosphere before updating your system version. You should also verify that the latest version of Atmosphere is compatible with the system version you're updating to. In this case, the latest version of Atmosphere is compatible with 11.0.1. Deleting the atmosphere folder from your SD card before updating Atmosphere is also a good idea.

If you're using fusee-primary (or you're chainloading fusee-primary using Hekate), use the fusee patches. If you're using Hekate, use the Hekate patches. Just one or the other.

Although most people don't do this, you can just use both .ZIP files if you don't know which one to use. There is no harm in having them together.
