your issue is

CMD is connected to DAT0. Or CLK is getting signal from one of the DAT lines.

Check if you mixed up the pins in the sd card adapter. especially pin 3 and 6. These pins are connected and sometimes one of them isn't even visible.

Since you are speaking of the ground, are you saying you think my ground may be causing the issue?

what have you soldered to those pins? Using one pin should be enough. they are connected inside the adapter.

Pin 3 is hidden => GND only soldered to pin 6! This could be the cause of your mixup.
Just follow the traces from the pads to the pins. Sometimes it's hard to see...

OR.. test with a multimeter to get the right pins to the pads.

I am just about at my wit's end here! lol

So, to test with mutimeter, I touch that PAD as well as the corresponding wire (from that diagram you linked) and make sure there is a connection. I tested all of the pads already.... only pads 3 and 6 are linked so there isn't anything crossing there, or shouldn't be.

Ok just tested with the multimeter... the correct pins seem to be at the correct pads... I am only getting a reading when I touch the corresponding wire to the corresponding pad. (sorry for the terrible terminology I am referring to touching positive and negative leads on the multimeter under the ohms setting)

Sounds good, I am convinced that the problem is in your adapter because the 3DS stops booting as soon as you attach the adapter to the 3DS. That shouldn't change anything...
My solderless mod is attached to a SD card by a luster terminal and it's working perfectly.

Did you solder the GND to 3&6? (splitted the wire?)

Just tested the wires soldered to the board. They also don't seem to have any corssing signals :/

Your soldering work is fine !

1. insert SD card
2. power ON -> Blue screen with error
3. plug to computer (doesn't warn you to format, it's normal too)
4. Do backup/restore

Also, just tried plugging in. Windows neither necognizes the card (which was stated coud be normal) nor does win32DiskImager

The bluescreen appearing when plugging the SD card (without the computer) is normal.
But not seeing the card on the computer is not.

either you mixed up the wires, or maybe they are too long, or big (thick).
Do you have pictures of your soldering work?

All is fine.

What I did for wiring is use two 3-pin computer fan cables (2 pins from each). This way I am able to physically disconnect the SD card adapter from the DS if I want to. When it is NOT plugged in it boots just fine. When it IS plugged in it blue screens whether the card reader is plugged into a PC or not.

I never had a blue screen while only the adapter was attached.
If I put the adapter in my SD reader and then turn on my 3DS, I have a blue screen, which is expected and good.
Then I plug the usb cable into my pc => drive popup.

The wires ar pretty thin. As far as length, I'd say in total they are less than ten inches (25cm)
