Homebrew What are all the files I need on an sd card?

My sd card just stopped working I put it in two laptop's with two different sd card readers (it's a micro) and it wants me to format and thinks it's in Raw format. It's been a long time since I've had to put files what all do I need? I can get nintendont back I think and I know to make an install folder. What else is required? I've got CBHC installed.
